ESG/MEGA (Endoscopic Sleeve Gastroplasty)

A minimally invasive endoscopic treatment for obesity designed to reduce procedural risk. Using an endoscope inserted through the mouth, the stomach is sutured from the inside to reduce its volume by approximately 70–80%. Because no abdominal incision is required, recovery is generally faster than with conventional surgery. Ablation therapy can also be performed during the same procedure.

Gastric Botulinum Toxin Injection

This treatment promotes weight loss by injecting botulinum toxin into the stomach wall. The toxin relaxes the stomach muscles, temporarily reducing gastric contractions and helping patients feel full sooner. The feeling of fullness also lasts longer than usual, helping to reduce food intake.

Ablation Therapy

Ablation therapy reduces stomach contractions and suppresses the secretion of ghrelin, the hormone that stimulates appetite. By decreasing the functional capacity of part of the stomach and lowering ghrelin levels, appetite is reduced, resulting in greater weight-loss effects than a single treatment alone.

Triple Approach Program

This comprehensive treatment combines ESG/MEGA, Ablation Therapy, and Gastric Botulinum Toxin Injection in a single procedure to maximize weight-loss outcomes.

Preoperative Assessment: Blood tests are performed, followed by a detailed explanation of the procedure by the physician (approximately 1 hour).
Day of Procedure: Performed as a same-day procedure. The treatment itself takes approximately 30–60 minutes, and patients can usually return home in as little as 3 hours, including the postoperative observation period.
The Following Day: A follow-up visit includes blood tests, examination of the treatment site, and a consultation with the physician.
Expected Outcomes: Average 15–20% total body weight reduction. When combined with anti-obesity medications, published studies have shown weight-loss results comparable to bariatric surgery. Systematic reviews conducted in Europe and North America have reported total body weight loss of 14.8–18.6%, with an average 15.9% weight reduction maintained at 5 years.
Gastric Botulinum Toxin Injection
The entire treatment is completed in a single visit.
After consultation with the physician, patients take pre-procedure medication and receive intravenous sedation. The procedure is completed in approximately 15 minutes while the patient is asleep. After 30–60 minutes of recovery and a final medical examination, patients may return home.
Overseas studies have reported an average 12% body weight reduction within 6 months, with reductions of up to 17% in some cases.
No preoperative or postoperative examinations or additional testing are required, other than the procedure day itself.

【Indications / Suitable Candidates】
ESG (Endoscopic Sleeve Gastroplasty)
Patients with a BMI of 27 or higher who have obesity-related comorbidities, such as diabetes (including prediabetes), hypertension, dyslipidemia, fatty liver disease, or obstructive sleep apnea.
Patients who wish to lose weight but are reluctant to undergo conventional bariatric surgery.
Busy individuals or those who repeatedly experience weight regain after dieting.
Patients with a BMI of 50 or higher, for whom conventional bariatric surgery carries increased risk, or those who have regained weight after previous bariatric surgery.

Gastric Botulinum Toxin Injection
Patients who have been unable to lose weight despite trying various dieting methods or who quickly regain weight after dieting.
Patients who have not achieved weight loss despite regular exercise, including training at a personal fitness gym.
Patients who have not achieved satisfactory results with weight-loss medications.
Patients who find it difficult to take oral medication every day.
Patients who prefer to avoid daily injections.

Precautions / Contraindications

If you proceed to the day of surgery without undergoing a preoperative gastroscopy (upper gastrointestinal endoscopy), the procedure may not be performed if the condition of your stomach is found to be unsuitable on the day of surgery.
If you are unable to communicate sufficiently in Japanese or English, you must be accompanied by an interpreter. A family member or friend may serve as your interpreter.
